DEFINITION OF A LARYNGECTOMY A total laryngectomy is the complete surgical removal of the larynx (voice box). In this procedure, the windpipe is brought up to the skin at the neck and a permanent stoma is created for the patient to breathe. The patient no longer has any upper airway and is solely breathing through their laryngectomy stoma site on their neck. DEFINITION OF A TRACHEOESOPHAGEAL PUNCTURE [TEP] A TEP is a small surgical puncture that is created through the back wall of the trachea into the esophagus. This puncture creates a connection between the trachea and the esophagus. A TEP is created for voice rehabilitation where a one-way valve prosthesis will be inserted into this puncture site and will allow the patient to produce speech. However, immediately after surgery a non-rigid foley catheter will be inserted into the TEP which will remain there for a minimum of 1 week postoperatively. This non-rigid foley catheter will keep the TEP open so that a prosthesis [VALVE] can be inserted at a later time. This non-rigid foley catheter will also provide a method of feeding for the patient immediately postoperatively. If the non-rigid foley catheter or prosthesis falls out of the TEP it must be reinserted immediately as the puncture site can begin to close within an hour or less. If the puncture site closes the patient will have to return to the operating room so that another TEP can be created.
